Remember, this card has a lot of important information so always carry it
with you. Your ID card has key components. First, this is your member ID number. It
allows us to identify you and the details of your specific plan. Right here
is where we show your plans primary subscriber or who’s actually paying for
the plan. If you’ve purchased an individual plan, your name will appear
here. If you have a family plan, your name will appear with the names of your
covered family members underneath it. Care type tells you the name of the
medical product you have. This is important to know because it describes
your plan benefits and lets us know who your network providers are, as well as
the level of coverage you have for prescriptions and doctor visits. Right
here you’ll find your care network. This network determines the doctors, hospitals, clinics and pharmacies we’ve contracted with to provide care at a discount to
you. Let’s flip your card over to the back. Now here you’ll find Medica’s
customer service phone numbers. This is a great resource to call if you have
questions. Our customer service phone lines are open Monday through Friday
during business hours. For medical concerns that aren’t urgent you can
